Residency Attestation Form (Spanish)
The attestation letter is intended for families who are unable to provide standard residency documents (such as a lease or proof of home ownership in the guardian’s name). If you are living with another resident, that individual will need to provide proof of address, the attestation form must be notarized and then uploaded with your online registration application.

Please note that additional documentation or verification may be requested to confirm residency.

This form must be completed each year if your living situation remains the same. Please ensure all information provided is complete and accurate.

If your family is experiencing housing instability, please indicate this in the online registration, as a different process may apply and support is available.

Affidavit of Enrollment and Residency 
To be used if you are an adult who has assumed responsibility for a child with a fixed night-time abode, for reasons other than access to the educational programs of the school district.

Appointment of Short-term Guardian Form
A parent or the guardian of the person of the child is appointing a guardian of a child of the parent (or a minor ward of the guardian, as the case maybe) NOTE: Permanent guardianship must be ordered within 60 days of short-term order. A separate form should be completed for each child. The person appointed as the guardian must sign the form, but need not do so at the same time as the parent or parents. This form is not accepted if the reason is to access educational programs of the school district.

Asthma Action Plan for Home and School
If your child has been diagnosed with asthma, please download this form and email, mbernero@fpsd91.org, or send it in with your student to the attention of the school health office.

Seizure Questionnaire
If your child has been diagnosed with seizures or epilepsy, please download this form and email, mbernero@fpsd91.org, or send it in with your student to the attention of the school health office.

Illinois Food and Other Allergy Emergency Action Plan and Treatment Authorization Form
If your child has been diagnosed with a life-threatening food/another allergy, please download this form and email, mbernero@fpsd91.org, or send it in with your student to the attention of the school health office.

Proof of School Dental Examination Form
Illinois law states all children in kindergarten and the second, sixth, and ninth grades of any school must have a dental examination submitted to the school no later than October 15 of the year. Please download the form and have your dentist complete it in order to have it ready to: a)upload during online registration; b) email to the school secretary; c) drop it off at your child’s school before the due date.

State of Illinois Eye Examination Report
Illinois law requires that proof of an eye examination be by an optometrist or physician be submitted to the school no later than October 15 of the year the child is first enrolled in school. Please download the form and have your dentist complete it in order to have it ready to: a)upload during online registration; b) email to the school secretary; c) drop it off at your child’s school before the due date.

Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations Form
This form must be used for students entering kindergarten or sixth grades.
A separate form must be completed for each child and for each vaccination/examination requested. The forms must be signed by a parent and the child’s health care provider and uploaded to the online registration form, emailed to the school secretary, or dropped off at the school.
